Peri-Implantitis and Failed Osseointegration

Peri-Implantitis is a destructive inflammation process that affects the gums and bone surrounding the implant. This is a form of gum disease in the bone that will make the implant become loose and may eventually cause it to fall out. In this case, the patient is then typically unable to have a new implant placed, because the bone has regressed past the point of successful intervention. It can occur when a patient who received dental implants has failed to maintain good oral hygiene and factors including diabetes mellitus, osteoporosis, smoking, long-term treatment with corticoids, radiation, and chemotherapy.

Osseointegration is the integration process that occurs once an implant is placed in the bone. This is when the bone and implant are so well integrated that the implant can withstand the forces of mastication (chewing). Essentially, failed osseointegration is the body rejecting the implant. 

Failed Osseointegration  can be caused by a multitude of different reasons for osseointegration failure  Underlying health issues like diabetes,  pre-existing gum disease, prescription medications, inadequate jaw bone, cancer and poor hygiene.

Concerning Signs

You will likely have follow-ups with your surgeon to ensure the implants are healthy during the initial healing period. During these appointments, your mouth will be examined for any concerning signs that may indicate that the implant is not healing properly.

Moving or shifting, infection in the mouth, and extreme pain or discomfort can all be causes for concern and should be discussed with your dentist immediately. If any of these problems arise and successful healing does not seem likely, your surgeon may remove your implant and offer another treatment method. 

How Long After Surgery Could My Implant Fail? 

While most dental implant failure occurs after the implant has been placed, failures or rejections can potentially happen up to ten years after the initial surgery. 

Other long-term risk factors can include excessive clenching and grinding, lack of bone or  gum tissue,poor oral hygiene for a long period of time and health factors as mentioned above.

Seeing a dentist regularly, maintaining good oral hygiene, and following up with your surgeon about any concerns can help to prolong the life of your dental implant. Once healed correctly and properly maintained, implants can be a permanent long-lasting choice for many patients.
